This includes any income received from SASSA grants, Unemployment Insurance Fund (UIF), or any retirement, life, disability&nbsp;or other benefits paid as a lump sum or in monthly payments.)<\/li>\n\n\n\n<li>A&nbsp;certified copy of your&nbsp;<strong>latest academic<\/strong>&nbsp;<strong>transcript<\/strong>&nbsp;or&nbsp;<strong>exam results<\/strong>. (If you are currently&nbsp;in&nbsp;<strong>Grade 12<\/strong>, you do not need to&nbsp;submit this)<\/li>\n\n\n\n<li>University registration documents (if applicable)<\/li>\n\n\n\n<li>Travel and Accommodation documents (if applicable)<\/li>\n\n\n\n<li>Proof of marital status (if applicable)<\/li>\n\n\n\n<li>Download and complete the\u00a0consent form\u00a0and fill it in with signatures of\u00a0all people whose incomes have been declared in the application.<\/li>\n\n\n\n<li>If you have a disability, download and complete the\u00a0Disability Annexure A Form<\/li>\n<\/ul>\n\n\n\n<p><strong>Manual Application Forms \/ Offline NSFAS Application:<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Visit one of the&nbsp;<a href=\"http:\/\/www.nsfas.org.za\/content\/application-centres.html\">application centers<\/a>&nbsp;where you will be given a hard copy of the application form to fill in.<\/li>\n\n\n\n<li>Use black ink to complete the nsfas application form and write within the blocks with CAPITAL letters.<\/li>\n\n\n\n<li>Do not cancel out any text and make sure to write clearly.<\/li>\n\n\n\n<li>Have parents\/guardian sign the consent form.<\/li>\n\n\n\n<li>Have certified copies of the listed supporting documents and attach them to the application form by stapling before submission.<\/li>\n\n\n\n<li>Documents must be certified on the front side of the page and should be within 3 months.<\/li>\n\n\n\n<li>Sign the completed application form and submit it to one of the NYDA assistants.<\/li>\n\n\n\n<li>The application will be attached with a barcode.
